How to Locate Survey and Form Answers

Learn where to find, review, and export survey and form answers from contact records or the Submissions area in Kyrios.

Who This Is For / When to Use

Use this guide if you need to view individual survey or form responses, export submissions to CSV, or analyze collected data.


Option 1: View Answers Within a Contact Record

Use this option when you want to see responses tied to a specific contact.

Access Additional Information

  1. Navigate to Contacts.

  2. Open the contact record.

  3. Expand Additional Info and any related folders.

  4. Review the stored survey and form answers.


Option 2: View Answers from the Form Submissions Area

Use this option to review, filter, and export all form submissions.


Step 1: Open the Submissions Page

  1. Navigate to Sites > Forms.

  2. Open a form.

  3. Click Submissions.


Step 2: Review Submission Details

  1. Click any submission row.

  2. View the submission panel on the right.

  3. Review contact details, answers, page source, and metadata.


Step 3: Search and Filter Submissions

  1. Use the search bar to find submissions by name, email, or phone.

  2. Apply a date range filter.

  3. Select the correct form or survey name.


Step 4: Download Submissions as CSV

You can export one or multiple submissions.

Download Selected Submissions

  1. Select one or more submission checkboxes.

  2. Click Download CSV.

Export Data as CSV

  1. Export submissions as a CSV file from the Submissions area.

  2. Open Google Sheets.

  3. Upload the CSV file.

This allows further analysis, sorting, and reporting.
