Rating Element for Forms, Surveys, and Quizzes

The Rating Element allows you to collect structured feedback using visual rating scales inside Forms, Surveys, and Quizzes.

Who This Is For / When to Use

Use the Rating Element when you need to:

  • Measure customer satisfaction or experience quality.

  • Collect scored feedback in surveys or quizzes.

  • Trigger automations or reporting based on numeric ratings.


Where to Find the Rating Element

The Rating Element is available inside the Form, Survey, and Quiz builder.

Steps:

  1. Open a Form, Survey, or Quiz.

  2. Click Edit.

  3. Open Quick Add in the left panel.

  4. Scroll to Rating.

  5. Drag the Rating element onto the canvas.


Adding the Rating Element

Once added, the Rating Element appears as a selectable block inside your form or survey.

Behavior:

  • Users select a rating by clicking an icon.

  • The selected value is stored with the submission.


Rating Settings

Rating-specific options are available in the right-side settings panel.

Configurable options:

  • Icon type (stars, hearts, thumbs-up, and more)

  • Icon alignment

  • Number of rating points

  • Lowest rating label

  • Highest rating label


Customizing the Rating Scale

You can control how many rating points are displayed.

Examples:

  • 1–5 star satisfaction rating

  • 1–10 numeric experience scale

Labels help clarify meaning for respondents.

Example labels:

  • Lowest rating: Bad

  • Highest rating: Good


Styling and Layout Controls

The Rating Element supports visual customization.

Available controls:

  • Icon alignment (left, center, top)

  • Spacing and padding

  • Consistent styling across desktop and mobile


How Ratings Are Stored

Ratings are saved as numeric values and linked to the submission.

Storage locations:

  • Submission records

  • Contact profiles

  • Contact activity timeline

  • PDF exports


Viewing Ratings in Contact Records

When a contact submits a rating, the value appears on their contact record.

Details:

  • Stored as a numeric value (example: 4)

  • Visible under the related Form, Survey, or Quiz entry


Viewing Ratings in Submissions

Ratings are also visible inside the Submissions view.

You can:

  • Review individual responses

  • Export submission data

  • See icon-based and numeric rating values


Mobile Responsiveness

The Rating Element is fully responsive.

Behavior:

  • Touch-friendly icons

  • Automatic layout adjustment for small screens


Common Issues and Fixes

Rating value not saving

  • Ensure the form or survey is published after changes.

Rating not visible in contact record
